This is an exciting opportunity to provide care across multiple agency partners and work in various home health settings. Clinicians are expected to carry out patient assessments, evaluations, and coordinate visits with support from a dedicated scheduling team.

Key Responsibilities of the Physical Therapist

• Perform higher-level visits such as SOC, evaluations, reassessments, and discharges
• Coordinate with PTAs for assistance with routine visits
• Document patient care within various EMR systems
• Utilize central scheduling platform for schedule visibility and support

Requirements of the Physical Therapist

• Physical Therapy license in Florida
• Experience in home health settings preferred
