About this role

Opportunity Highlights

At Trinity Health Mid-Atlantic, physicians are empowered to make a difference where it matters most. Join a mission-driven organization dedicated to expanding access, addressing health disparities, and improving the well-being of the communities we serve. Here, you'll find the resources of a leading health system, the support of collaborative colleagues, and the opportunity to build a legacy of compassionate care that extends far beyond individual patients.

Primary Care-Cottman, part of Trinity Health Mid-Atlantic, is seeking a Board-Certified or Board-Eligible Primary Care Physician to support continued primary care expansion and access initiatives across the Philadelphia market. This position offers a 100% outpatient setting, strong clinical support, and the ability to build a patient panel within a mission-driven, community-focused health system.

The Practice

  • Established, high-demand primary care practice
  • Immediate patient volume available
  • 100% outpatient setting with no inpatient responsibilities
  • Team-based care model with advanced practice providers and support staff
  • Integrated Epic electronic health record
  • Strong referral network within Trinity Health Mid-Atlantic Medical Group

Key Responsibilities

  • Complex Chronic Disease Management: Diagnose and manage advanced, often co-occurring chronic conditions such as poorly controlled diabetes, severe hypertension, substance use disorders, and asthma exacerbated by urban environmental factors
  • Social Determinants of Health Intervention: Screen for and address non-medical barriers to health, including food insecurity, housing instability, transportation deficits, and low health literacy
  • Trauma-Informed Care: Utilize trauma-informed communication strategies to care for patients experiencing chronic stress, chronic illness, or historical medical mistrust
  • Interdisciplinary Care Coordination: Collaborate closely with on-site social workers, case managers, community health workers, and behavioral health specialists to provide holistic patient support
  • Preventive Medicine and Harm Reduction: Administer critical immunizations, perform routine health screenings, and implement harm reduction strategies for high-risk behaviors
  • Resource Optimization: Navigate financial barriers by utilizing sliding fee scales, prescribing low-cost formulary medications, and leveraging local public health assistance programs
  • Electronic Health Record Documentation: Maintain thorough and timely documentation in the integrated Epic system

Schedule and Call Responsibilities

  • Full-time outpatient schedule with 34 patient-facing hours per week
  • Flexible scheduling options available
  • Minimal call responsibilities on a 1:6 call schedule, phone-only coverage
