Schedule: Monday through Friday 10am - 6pm

Salary: $64,000–$69,000 

The JDP Post Response (PR) Clinician will provide a clinically appropriate array of services including outreach, crisis and interim counseling, psychoeducation, care coordination/case management, advocacy, and occasional transportation to individuals and families involved with JDP. JDP PR Clinician will work with program participants to assess mental health, substance use, risk level, and strengths and needs. JDP PR Clinician will work collaboratively with participants to develop and implement person-centered treatment plans that will support achievement of participants’ goals, reduce risk and barriers, and support engagement in appropriate levels of care.

Responsibilities

• Provide program participants with crisis and interim counseling, psychoeducation, care coordination/case management, advocacy, occasional transportation, and other necessary support services.
• Demonstrate cognitive and affective flexibility to effectively meet the unique needs of each participant.
• Deliver care and support in alignment with The Advocates Way and uphold its core values in all interactions.
• Promote a strengths-based, client-driven approach by empowering participants in conflict resolution and informed decision-making.
• Utilize evidence-based practices to complete required assessments, provide risk and interim counseling, and support care coordination.
• Identify appropriate internal and external support services and facilitate supported referrals to community resources as needed.
• Develop, implement, and maintain person-centered treatment plans; meet regularly with assigned participants to monitor, evaluate, and document progress in accordance with program requirements.
• Conduct initial intake meetings and assess participant needs to inform service planning and resource development.
• Facilitate timely referrals to appropriate levels of care or external providers in collaboration with the clinical team.
• Ensure prompt connection to emergency services or crisis intervention resources when required.
• Collaborate with the Post Response Supervisor and designated clinical staff to interpret assessment results, reassess service needs, and identify appropriate community-based supports aligned with participant goals.
• Work with supervisory and program staff to evaluate program effectiveness, identify areas for improvement, and adjust workflows and caseloads as needed.
• Coordinate communication with partner agencies and multidisciplinary providers, including clinical staff, probation officers, and other stakeholders.
• Establish and maintain professional, collaborative relationships with internal team members and external partners, including Co-Response and law enforcement agencies when appropriate.
• Accurately document all participant interactions, services, and progress toward treatment goals in accordance with program standards.
• Maintain up-to-date records, including data, notes, and required documentation, using electronic health record systems and other tools; participate in regular case reviews with supervisory staff.
• Complete all documentation, including notes, updates, and assessments, within established timeframes and in accordance with agency, funder, and regulatory requirements.
• Provide transportation to participants when clinically appropriate, approved by supervisory staff, and in accordance with agency transportation policies.
• Maintain appropriate professional boundaries with participants, staff, and community partners.
• Participate actively in supervision, staff meetings, multidisciplinary team meetings, and other relevant forums.
• Complete all required trainings and maintain current certifications.
• Maintain working knowledge of managed care and health insurance systems and share relevant information and resources with staff and providers.
• Remain attentive and engaged throughout scheduled shifts to ensure participant safety and service quality.
• Uphold all principles of The Advocates Way and ensure participants are treated with dignity and respect in accordance with the agency’s Human Rights policy.
• Perform all duties in compliance with agency policies, procedures, and performance standards.
• This role requires travel throughout the Blackstone Valley.

Requirements

• Master’s degree in social work or in human services or health care related field with at least one year minimum experience working with individuals in a crisis or community-based support setting.
• Ability to develop and maintain satisfactory working relationships with participants and community providers.
• High energy level, superior interpersonal skills and ability to function in team and independent atmospheres.
• Must adhere to all professional ethics.
• Ability to communicate effectively verbally and in writing.
• Must have basic computer skills.
• Ability to read and speak English.

• Strongly prefer that a candidate will have a demonstrated understanding of and competence in serving culturally diverse populations.
• Must hold valid driver’s license and access to an operational and insured vehicle.
• Must be able to perform each essential duty satisfactorily.
