APT | Licensed Physical Therapist Chelsea, IA $88,999 - $116,999 a year Job Type: Full Time We are seeking a compassionate and skilled licensed physical therapist to join our team in a regional healthcare facility serving the Midwest. The primary responsibility is to evaluate, examine, treat, and continuously assess patients to improve their physical function and quality of life. Key Responsibilities: Conduct thorough patient evaluations and develop individualized treatment plans using approved examination and evaluation procedures. Communicate treatment plans effectively to patients, families, caregivers, and the interdisciplinary care team. Complete all required documentation including evaluations, daily notes, progress reports, updated care plans, discharge summaries, and logs in compliance with federal, state, and facility regulations. Collaborate with rehabilitation team members to coordinate comprehensive care and select appropriate treatment programs tailored to patient needs. Create and implement home exercise programs to support patient progress outside of therapy sessions. Develop evidence-based treatment protocols and discharge recommendations aligned with patient-specific goals. Respond promptly to referrals and maintain strong professional relationships with referring healthcare providers. Maintain a positive, service-oriented approach with patients, families, caregivers, and colleagues at all times. Monitor patient progress and adjust treatment plans as necessary to ensure effective outcomes. Train and supervise physical therapy assistants and support staff in accordance with federal and state regulations. Assist with equipment maintenance and manage supply inventory. Perform other duties as assigned to support department operations. Education and Certification: Bachelor's, Master's, or Doctorate degree in Physical Therapy (BA, BS, MA, MS, or DPT). Current physical therapy licensure and board certification as required by state regulations. Up-to-date Basic Life Support (BLS) certification following American Heart Association standards. Completion of Mandatory Reporter training within six months of hire if not already completed. Compliance with all federal, state, and professional licensure and regulatory training requirements.
