Pilsen Medical Clinic is currently seeking a qualified Certified Medical Assistant to join their Team. This is an in office position. Bi-lingual in Spanish is preferred for this position. Responsibilities: Apply critical thinking skills and problem-solving techniques to the chronic care process. Maintain current knowledge of disease processes, resources, and treatment options available. Meet performance goals. HIPAA compliant use of computer access to facilitate patient care. Use care plan to guide patient interaction and direction. Review individual care plans and make phone calls to CCM enrolled patients. Calls may include: Assess current health status Medication reconciliation & compliance assessment Appointment reminders Appropriate patient education re: care & condition management at home Review orders, labs, consults and associated documentation Encourage preventative health services Appointment reminders Assists in the assessment of patient needs and health by taking patient history, medication listing, and assessing patient’s physical and mental status. Completes all necessary documentation in the patient’s medical record in accordance with the practice’s charting policy. Requirements: Basic understanding of medical terminology. Understanding of documentation required for Chronic Care Management. Ability to understand and carry out written, oral and/or graphic instructions. Ability to multi-task, manage details and organize efficiently and effectively. Ability to interact with patients, medical and administrative staff, and the public effectively. EMR experience Minimum 1-year experience in home-care or clinical setting. Intermediate proficiency with computers and telephone use Quickly recognize condition-related warning signs. Organized, thorough documentation skills. Self-directed. Ability to prioritize responsibilities. Demonstrated time management skills. Positive and proactive attitude; team player! Goal orientated Must be punctual and have reliable transportation Must have a valid form of healthcare certification and/or license (CMA) Bilingual candidate preferred. Job Type: Full-timePay: $18.00 - $19.00 per hour Job Type: Full-time Schedule: Monday to Friday Education: High school or equivalent (Preferred) License/Certification: BLS Certification (Preferred) Certified Medical Assistant (Preferred) Work Location: In person
