Description- Conduct patient assessments within both inpatient and outpatient environments and create tailored treatment strategies
- Deliver physical therapy treatments aimed at improving mobility, strength, balance, and overall function
- Collaborate closely with nurses, doctors, case managers, and other rehabilitation specialists to enhance patient outcomes
- Provide education to patients and their families about therapy goals, discharge processes, and home exercises
- Ensure accurate documentation of evaluations, treatments, and patient progress per facility guidelines
- Take part in multidisciplinary care team rounds and conferences to promote integrated care
- Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution
- Must possess state licensure as a Physical Therapist
- CPR certification is preferred
- Additional certifications (e.g., specializations in neurology or geriatrics) are advantageous but not mandatory
- Prior experience is strongly desired
- Ability to provide care for patients with intricate medical and mobility challenges
