Job ResponsibilitiesOur organization is in search of an enthusiastic and mission-oriented Lead Physician for Primary Care. This role is pivotal in implementing team-based collaborative care and enhancing population health for our medically complex and socially vulnerable members.
We focus on providing care to individuals facing significant medical challenges and social vulnerabilities. Our approach transitions care from emergency rooms to a comprehensive, patient-centered setting that emphasizes dignity, accessibility, and holistic support.

- Deliver exceptional primary care to a designated panel of members, leading your interdisciplinary care team.
- Guide fellow providers in delivering high-quality primary, urgent, and community care, while effectively leading their interdisciplinary teams. Mentor care teams during population health meetings to meet quality and utilization objectives.
- Serve as the clinical leader alongside the Medical Director, addressing inquiries and providing clinical guidance to nurses, medical assistants, and other providers regarding clinical cases as they arise.
- Be an integral part of the center and market leadership team, responsible for steering the market towards achieving its quality, utilization, and engagement targets. Participate in leadership meetings and represent our organization to community organizations, hospital systems, and payer partners.
- Deliver high-quality primary care, managing chronic conditions, acute exacerbations, and follow-up needs post-hospitalization or ER visits.
- Collaborate with an interdisciplinary team (including behavioral health, nursing, social work, pharmacy, nutrition, and care management) to develop and modify patient care plans.
- Coach and mentor physicians and Advanced Practice Providers on best practices in value-based and evidence-based care.
- Evaluate when patients can be safely treated within our organization and when higher-level care is necessary.
- Accept same-day, next-day, and urgent appointments for members requiring intensified physician or care management intervention to avert admissions or readmissions.
- Monitor patient health trends and proactively implement strategies to decrease ER visits, hospitalizations, and readmissions.
- Practice evidence-based medicine in alignment with Patient-Centered Medical Home (PCMH) principles, ensuring quality, prevention, and continuity of care.
- Develop and oversee transitional care plans, tracking patients throughout the entire continuum of care.
- Achieve and sustain quality metric goals, utilizing data and electronic health record tools to enhance performance and efficiency.
- Provide on-call coverage on a rotating basis.
- Allocate approximately 70-80% of your time to direct clinical care.
- Perform additional duties related to chronic care and disease management services as required.
- Possession of an active, unrestricted medical license in the state of employment.
- Clinical and administrative experience in leading interdisciplinary teams.
- Experience in a clinic, community-based organization, or inpatient unit.
- Background in training and mentoring others.
- Strong problem-solving, project management, and organizational skills.
- Self-motivated, independent thinker with an interest in population health, healthcare utilization management, and the continuous improvement of our care model.
Monday to Friday, from 8 AM to 5 PM.
