Practice & Cases

• Outpatient clinic focused on preventive and chronic care
• Management of diabetes, hypertension, CHF, and heart disease
• No procedures required
• Team-based clinic model with full clinical support staff

Schedule

• Monday through Friday clinic schedule
• Daytime hours from 8am to 5pm
• Full-time outpatient coverage
• Ongoing assignment with repeat weekday blocks

Requirements

• Must be an MD or DO
• Board Certified in Family Medicine or Internal Medicine
• Active Illinois license required
• DEA required

Logistics

• Mileage reimbursed
• Drive-in candidates preferred
• Lodging considered on a case-by-case basis
