This role is ideal for an independent practitioner who enjoys working with the geriatric population and building lasting relationships with residents while delivering high-quality primary care. Position Options Part-Time 2–3 full days per week Travel between Hoosick Falls, NY and Cambridge, NY Full-Time 4–5 full days per week Travel between Hoosick Falls, NY, Cambridge, NY, and Troy, NY Requirements Required Active New York State Nurse Practitioner License Active DEA Registration Currently enrolled and credentialed with Medicare and Medicaid Minimum of 3,600 clinical practice hours allowing independent practice under New York State regulations Primary Care experience Preferred Geriatric Care experience Assisted Living or Long-Term Care experience Benefits Compensation & Benefits Competitive compensation package $5,000 retention bonus after 90 days Flexible scheduling options Supportive clinical environment Anticipated start date within 4–6 weeks
