Description

Human Touch Home Health is seeking a licensed Physical Therapist (Per Diem) to provide home health services throughout Fairfax County, Loudoun County, and Prince William County, Virginia.

What You'll Do

• Complete evaluations, treatment plans, reassessments, and discharge planning
• Provide skilled physical therapy services in patients' homes
• Coordinate care with physicians and the interdisciplinary team
• Manage your own schedule and service area

Requirements

• Active Virginia Physical Therapist license
• Valid driver's license and reliable transportation
• Strong documentation and communication skills
• Home health experience preferred
