Beadles Nursing Home has cared for residents in Alva, Oklahoma for generations. We are a fifth-generation family owned skilled nursing facilities serving around 50 residents at Beadles Nursing Home and another 30 at Beadles New Beginnings.

The Role
We are hiring a Physical Therapist to lead our rehabilitation program as the Director of Rehab. You will be the sole therapy clinician across both buildings, which means this program is yours to shape. You will set the pace, build the relationships, and decide how rehab best serves our residents.
Your responsibilities will include:
- Evaluating residents and developing individualized plans of care
- Providing skilled physical therapy services across both facilities
- Directing and training restorative nursing staff
- Fitting DME and assistive devices
- Participating in care planning meetings
- Educating staff on safety practices that protect both residents and workers
- Communicating and coordinating across departments to support whole-person care
- Contributing to staff wellness initiatives
- Building the rehab program in a way that reflects our mission
Who We Are Looking For
The right person for this role brings:
- An active Oklahoma physical therapy license, or eligibility through the PT Compact or Oklahoma licensure
- Experience in skilled nursing or long-term care
- Working understanding of PDPM, Medicare Part A and Part B documentation, and SNF rehab regulations
- Comfort supervising restorative nursing and working independently
- Genuine respect for older adults and the people who care for them
- Strong communication with families, staff, and our team
- A heart for this kind of work

Compensation and Benefits
- $55 to $70 per hour, based on experience
