We are currently looking for a committed Primary Care Physician to become part of our multi-specialty team at the innovative Health Park in Mays Landing. This role presents an exciting chance to provide high-quality, patient-focused care while contributing to the evolution of healthcare in our community.

Practice Highlights & Innovations

Integrated, Team-Based Care:

• RN Care Managers: Assist all providers in managing risk-stratified patient populations and ensuring coordinated care for those most in need.
• Embedded Behavioral Health: Onsite support from Social Workers, Licensed Clinical Workers, Psychiatrists, and Advanced Practice Nurses to offer comprehensive behavioral health services.
• Collaborative Hospitalist Support: Ensure smooth transitions and support for patients requiring inpatient care.

Accessible & Patient-Centered:

• In-Person and Virtual Care: Provide patients with flexible options, including both on-site and full-time virtual primary care, with same or next-day appointments available seven days a week.
• Comprehensive Services: On-site laboratory and community pharmacy for enhanced convenience and efficiency.

Dedication to Health Equity & Community:

• Serve diverse populations through our FQHC locations, all recognized as Patient-Centered Medical Homes (PCMH), offering sliding-scale fees and care for everyone, regardless of their ability to pay.
