Details Home health setting serving patients across a multi-city territory including San Jose, Milpitas, Saratoga, Los Gatos, Campbell, Morgan Hill, Gilroy, Sunnyvale, and surrounding communities Day shift schedule from 8:00 AM to 5:30 PM, with flexibility based on patient availability One weekend day required per month 26-week contract Requirements Minimum 1 year of home health PT experience required Active state PT license required (license required if applicable) Current BLS/BCLS certification required Experience with OASIS documentation required Additional Information Responsibilities include evaluating and treating patients using medically prescribed physical therapy programs to restore function and maximize patient performance May supervise Physical Therapy Assistants as part of the care team Responsible for completing initial admission and evaluation visits and reporting OASIS scores to the Clinical Team Manager within established timelines Documentation duties include recording clinical findings, skilled interventions, goals, and discharge plans American Traveler Job ID P-712913.
