Seven Healthcare are seeking an experienced Physical Therapist for a travel assignment in New York, NY. This role requires Oasis experience and involves providing case management and direct patient care as part of the Monroe County team. The successful candidate will administer comprehensive physical therapy modalities within the home setting.

Key Responsibilities of the Physical Therapist
- Develop and revise treatment and discharge plans in collaboration with patients, families, and medical professionals.
- Initiate rehabilitative programs and supervise PTA to ensure patient health, safety, and plan compliance.
- Consult with and educate patients and families regarding disease processes and self-care techniques.
- Implement and adjust home therapy programs based on patient progress evaluations.
- Coordinate and supervise quality patient care services provided by the Home Health Agency.
- Participate in patient conferences to discuss care plans and ensure coordinated treatment.
- Ensure the completion of clinical records for state and federal compliance.
Requirements of the Physical Therapist
- A B.S., M.S., or D.P.T. degree in Physical Therapy
- Current New York State License in Physical Therapy
- BLS (AHA only)
- Minimum of one year experience in a Long Term Care/Home Care setting
- Recent Home Care experience (within the last year)
- Oasis experience required
