Physical Therapist Location: Dallas, GA Agency: Cynet Health Pay: $2,426 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 6/22/2026

Job Title: Physical Therapist Profession: Therapy/Rehabilitation Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 full year of adult home health experience required License: Must have a current license in hand Certifications: CPR, BLS Must-Have: Reliable transportation, current driver's license, and agency-required liability insurance Description: Home visits will be required for this position. Periodic office meetings are expected. Contractor will need to travel up to 75 miles from the home clinic. Mileage will be reimbursed at the IRS standard rate. The contractor is compensated from patient home to patient home. The commute to and from the first and last patient is not billable. Contractor will be assigned between 5 to 10 patients per day. The average number of patients per day is 5 to 6. Daily productivity expectations will be hitting 30 points per week. Each type of visit and activity (documentation, travel, etc.) holds a different point value. Types of cases include Start of Care and Follow-Ups. Weekend requirements may vary per location and profession; this should be discussed during the interview process. Different job locations may have varying on-call requirements, please discuss with the manager during the interview. All documentation must be completed within 24 hours of the patient appointment. Delayed and incomplete documentation that impacts billing and follow-up patient care will result in cancellation. All contractors must be open to floating to another nearby clinic if patient volume is low. No guaranteed hours are provided with this position. The contractor must accept every patient on their schedule regardless of distance from previous appointments. Preference will be given to those with recent home health experience. This position does not require a COVID-19 vaccine. Flu vaccine status is unknown.
