Physical Therapist Location: Sunnyvale, CA Agency: Jackson Therapy Partners Pay: $2,605 to $2,805 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: ASAP About the Position Looking for a change of scenery and a chance to help others. Join as a traveling Physical Therapist providing home health services in Sunnyvale, CA. You will evaluate and treat patients using medically prescribed physical therapy programs, document clinical findings and treatment plans, and may supervise Physical Therapy Assistants. The role involves initial admission/evaluation visits and timely communication of OASIS scores to the Clinical Team Manager. This position supports restoring function, preventing disability, and helping clients reach maximum performance within their capabilities. Minimum Requirements Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA. Active California physical therapist license required to start the assignment. BLS certification from AHA or ARC may be required. Experience in home health settings preferred. Ability to perform initial admission/evaluation visits and document OASIS assessments within established timelines preferred. Preferred Qualifications Previous supervisory experience with Physical Therapy Assistants preferred. Experience managing caseloads across multiple territories preferred (example: South San Francisco, Pacifica, Millbrae, San Mateo, Redwood City). Assignment Details Shift Hours: 8:00 am - 5:30 pm (schedule can be flexible based on patient availability). Territory may include South San Francisco, Pacifica, Millbrae, Hillsborough, Burlingame, San Carlos, Foster City, San Mateo, Redwood City and surrounding areas.
