Physical Therapist Location: Sunnyvale, CA Agency: Jackson Therapy Partners Pay: $2,605 to $2,805 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 26 Weeks Start Date: ASAP

Looking for a change of scenery and a chance to help others. Join a traveling Physical Therapist team supporting home health patients in Sunnyvale, CA. The Physical Therapist will evaluate and treat patients to restore function, prevent or minimize disability, and help clients reach maximum performance within their capabilities. Responsibilities include initial admission/evaluation visits, documenting observations, clinical findings, problems, skilled interventions/treatments, goals and discharge plans, and notifying the Clinical Team Manager of OASIS scores within established timelines. The clinician may supervise Physical Therapy Assistants and perform other duties consistent with skills and training. Thank you for your time and responsiveness.

Minimum Qualifications

• Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active California Physical Therapy license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Home health experience preferred, including conducting initial admission/evaluation visits and OASIS documentation.
• Experience supervising Physical Therapy Assistants preferred.
• Ability to manage a caseload across multiple service areas within the assigned territory.

Preferred Qualifications

• Prior travel or per-diem home health experience preferred.
• Experience with documentation systems used in home health settings preferred.

Assignment Details

• Shift hours: 8:00 AM to 5:30 PM. Schedule can be flexible based on patient availability.
• Weekend requirement: One weekend day per month.
• Territory includes communities such as Milpitas, Saratoga, Monte Sereno, Los Gatos, Campbell, San Jose, Morgan Hill, San Martin, Gilroy, and nearby areas. Candidate may travel within this territory to provide patient care.
