Physical Therapist Location: Newton Center, MA Agency: Cynet Health Pay: $2,748 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 6/28/2026 About the Position Job Title: Physical Therapist Home Health Profession: Physical Therapist Specialty: Home Health Duration: 13 weeks Shift: 08:00 AM - 04:30 PM Hours per Shift: 8 Experience: Minimum 1 year of Certified Home Care experience required. License: Active Physical Therapist License Certifications: CPR Certification Must-Have: - 1 year Home Health experience - 1 year Case Management experience within a certified home health agency Description: All applicants must have 1 year of Certified Home Care experience. 1 year of Case Management experience is required. Experience in hospice, private care, or working as a Case Manager on its own does not meet the qualifications for Certified Home Health Care Management experience. Only roles specifically within a certified home health agency will be considered relevant for the 1-year requirement. Ability to travel to assigned cases is required. Weekend rotations are once every 6 weeks. Responsibilities include assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families. Patient care is delivered within organizational philosophy, policy, and standards of community health practice. Also required to perform admission processes and case management responsibilities in accordance with policies and standards of practice.
