About the Position**Job Title: Home Health Physical Therapist**
**Location:** Southborough, MA
**Job Description:**
We are seeking a Home Health Physical Therapist for a 13•week contract, Monday to Friday, 8:00 AM to 4:30 PM. Provide quality care to patients in their homes, manage cases, and collaborate with healthcare professionals.
**Key Responsibilities:**
• Deliver physical therapy services to home health patients.
• Complete documentation after each visit.
• Manage daily cases, including Start of Care and at least four visits.
• Collaborate with healthcare team for comprehensive care.
**Requirements:**
• Valid MA Physical Therapist License.
• Basic Life Support (BLS) certification.
• 2+ years of home health experience.
• Familiarity with EMR, especially Home Care Home Base, preferred.
**Additional Information:**
• No on•call or holiday duties.
• Possible floating to other locations if needed.
• COVID•19 vaccination not required.
