Job Title: Home Health Physical Therapist Profession: Operations Specialty: Home-Hospice Care Operations Duration: 13 weeks Shift: Day Shift Hours per Shift: 8 Experience: Minimum 2 years Home Health experience with Adult/Geriatric patients License: Active Physical Therapist license (IL) Certifications: BLS (AHA), valid Driver's License, and auto insurance Must-Have: - Must have 2 years of Home Health experience with Adult/Geriatric patients - Must have an active Physical Therapist license (IL) and BLS certification - Must provide reliable transportation Description: The traveler can expect to be onsite for orientation for approximately 2 days. The position is 100% remote within the designated coverage area. There is no physical office to report to, as all meetings are held virtually. The current census for this team is approximately 200 patients. Patients will mainly be adult/geriatric general medical/surgical type patients. The expectation is to see 6 patient visits per day within the coverage area. Supplies and equipment will be delivered to the traveler's home. The candidate must provide their own transportation to and from all patient visits. Attire can consist of any solid color scrubs or business casual; jeans and open-toed shoes are not allowed. The coverage area includes specific ZIP codes within the Central Metro area. The candidate must assist with Activities of Daily Living (ADLs) and adaptive equipment. The candidate should be able to effectively collaborate with other specialties such as RN, OT, Speech, etc. The expected patient ratio is 1:1, with 6 patients to be seen daily. The position does not involve weekend rotations, on-call duties, or holiday expectations.
