Floating Requirements within Central Metro E coverage area Position Overview

Seeking an experienced Home Health Physical Therapist (PT) to provide care for adult and geriatric patients within the Central Metro E coverage area of Chicago. This is a field-based role with virtual team collaboration. Travelers will complete approximately 2 days of onsite orientation in Oak Brook, IL with Advocate@Home before transitioning to a fully remote field-based schedule within the assigned territory.

Patient Population

• Adult and geriatric general Med/Surg patients
• Current team census: approximately 200 patients
• Expected productivity: 5–6 patient visits per day
• 1:1 patient care

Required Qualifications

• Minimum 2 years of Home Health PT experience
• Experience with adult/geriatric patients
• Active Illinois PT License
• BLS Certification (AHA required)
• Master's Degree in Physical Therapy
• Valid Driver's License
• Active auto insurance and reliable transportation
• Recent Home Health experience strongly preferred
• First-time travelers considered with strong experience

Required Skills

• Home care/Home Health experience
• Oasis documentation experience
• EPIC EMR experience
• Ability to assist with ADLs and adaptive equipment
• Strong interdisciplinary collaboration skills with RN, OT, Speech, and other care team members

Technology & Equipment

• EMR: EPIC
• IV Pumps: Sapphire
• Supplies and equipment delivered to traveler's home

Orientation

• Approximately 2 days onsite in Oak Brook, IL
• Includes EMR training and skills check-off

Scheduling & Call

• Monday–Friday schedule
• No weekends
• No call
• No holiday requirements

Dress Code

• Any solid-color scrubs or business casual attire
• No jeans or open-toe shoes

Additional Notes

• This is a fully remote field-based position after orientation
• No physical office reporting location
• All meetings conducted virtually
• Traveler must provide transportation to/from all patient visits
