Contract | Days | Monday through Friday, 8:00 AM to 5:00 PM | 13-Week Assignment

Key Responsibilities

• Conduct Start of Care evaluations and follow-up visits for home health patients within an assigned territory
• Meet daily productivity expectations through a combination of Start of Care and follow-up visits
• Document all patient care activities accurately and in a timely manner using home health electronic medical record software
• Develop and implement individualized physical therapy treatment plans in the home setting
• Collaborate with the interdisciplinary care team to ensure coordinated, patient-centered care

Required Qualifications

• Active Physical Therapist (PT) license in the state of California
• Experience in a home health setting
• Proficiency with HCHB (Homecare Homebase) electronic medical records system
• Valid driver's license and reliable transportation for home visits within the assigned territory

Preferred Experience / Skills

• Familiarity with the Santa Clarita/Valencia area
• Strong time management skills with the ability to consistently meet daily productivity goals
