Contract | Days | Monday–Friday | 32–40 hours per week | Outpatient
Key Responsibilities
- Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction.
- Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines.
- Delegate and direct discipline-specific therapy services for assigned patients, ensuring design of functional treatment programs that support effective return to community activities.
- Serve as an educator for patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations as assigned.
- Adhere to discipline-specific State Practice Act and all applicable regulatory requirements, maintaining professional licensure in good standing and completing all state-mandated continuing education requirements.
- Participate in performance improvement initiatives, offering feedback and applying learned information to meet organizational quality objectives.
Required Qualifications
- Active and unrestricted Physical Therapist license in the state of Georgia (or eligibility to obtain).
- Ability to complete comprehensive patient evaluations and develop evidence-based treatment plans and goals.
- Proficiency in medical record documentation in compliance with state, federal, and payer guidelines.
- Commitment to patient and employee safety standards, including safe patient handling and infection control practices.
- Ability to adhere to professional codes of conduct and maintain ethical practice standards.
Preferred Experience / Skills
- Prior experience in an outpatient physical therapy setting.
- Experience with student supervision and clinical education programs.
- Demonstrated involvement in continuing education and professional development activities.
