Key Responsibilities
- Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in alignment with patient needs, goals, and physician direction
- Conduct 60-minute initial evaluations and 30-minute follow-up sessions in an outpatient setting
- Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
- Design functional treatment programs focused on progressing patients toward discharge and effective return to community activities
- Delegate and direct discipline-specific therapy services for assigned patients
- Serve as an educator for patients, families, students, and coworkers; participate in student programs as assigned, including supervision, development of learning objectives, and completion of student evaluations
Required Qualifications
- Active and unrestricted Physical Therapist licensure in good standing in the applicable state
- Adherence to discipline-specific State Practice Act and all relevant regulatory agency requirements
- Completion of all state-mandated continuing education requirements
- Ability to perform in an ethical manner in accordance with the discipline-specific professional code of conduct
- Demonstrated competency in clinical reasoning, treatment planning, and functional goal writing
Preferred Experience / Skills
- Prior experience in an outpatient physical therapy setting
- Experience with student supervision and clinical education programs
- Familiarity with performance improvement methodologies and quality of care initiatives
