Key Responsibilities

• Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in alignment with patient needs, goals, and physician direction
• Conduct 60-minute initial evaluations and 30-minute follow-up sessions in an outpatient setting
• Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
• Design functional treatment programs focused on progressing patients toward discharge and effective return to community activities
• Delegate and direct discipline-specific therapy services for assigned patients
• Serve as an educator for patients, families, students, and coworkers; participate in student programs as assigned, including supervision, development of learning objectives, and completion of student evaluations

Required Qualifications

• Active and unrestricted Physical Therapist licensure in good standing in the applicable state
• Adherence to discipline-specific State Practice Act and all relevant regulatory agency requirements
• Completion of all state-mandated continuing education requirements
• Ability to perform in an ethical manner in accordance with the discipline-specific professional code of conduct
• Demonstrated competency in clinical reasoning, treatment planning, and functional goal writing

Preferred Experience / Skills

• Prior experience in an outpatient physical therapy setting
• Experience with student supervision and clinical education programs
• Familiarity with performance improvement methodologies and quality of care initiatives
