The Role
Our client is seeking a licensed Physical Therapist for a full-time permanent position within an outpatient clinic setting. The role involves comprehensive evaluation and treatment of patients, individualized care plan development, and ongoing collaboration with clinical staff to ensure consistent, high-quality patient outcomes. This is a direct hire opportunity working a 4-day, 10-hour shift schedule.
Key Responsibilities
- Evaluate patients and develop individualized outpatient physical therapy treatment plans
- Provide direct outpatient physical therapy services per plan of care
- Monitor patient progress and adjust treatment plans as clinically indicated
- Maintain accurate documentation and ensure compliance with applicable standards
- Collaborate with clinical staff across disciplines to support care coordination
Requirements
Must-Have
Active Arizona Physical Therapist (PT) License current and in good standing
Minimum Master s Degree in Physical Therapy or related field
Minimum 5 years of physical therapy experience (outpatient experience required)
Strong communication and patient care skills
Preferred / Nice-to-Have
- BLS Certification current
- Prior outpatient clinic experience in a tribal health or federally qualified health setting
