Key Responsibilities

• Provide physical therapy evaluation and treatment services within a skilled nursing facility (SNF) setting
• Deliver patient care across assigned worksites, with openness to floating between locations as needed
• Maintain accurate and timely clinical documentation in accordance with facility and regulatory standards
• Collaborate with interdisciplinary care teams to support patient rehabilitation goals
• Adhere to all credentialing, compliance, and licensure requirements throughout the assignment

Required Qualifications

• Active Physical Therapist license in the state of California
• Minimum of 1 year of experience in a skilled nursing facility (SNF) setting
• Willingness to float between worksites as operationally required
• Ability to work 8-hour day shifts, 5 days per week

Preferred Experience / Skills

• Experience using Casamba clinical documentation software
• Familiarity with SNF workflows, productivity standards, and Medicare documentation requirements
