Physical Therapist Location: Marion, OH Agency: Concentric Healthcare Staffing Pay: Competitive weekly pay (inquire for details) Shift Information: 8 hours Contract Duration: 13 Weeks Start Date: ASAP About the Position Job Title Physical Therapist (PT) Location Marion, OH Schedule / Job Type Contract | Days | Monday – Friday, 8:00 AM – 5:00 PM | On-Call Required Key Responsibilities Provide physical therapy services to home health patients in accordance with plan of care Complete OASIS assessments as required within the home health setting Document patient care accurately and timely using HCHB software Meet productivity expectations of 30 points per week Participate in on-call rotation as required Required Qualifications Active Physical Therapist (PT) license in the state of Ohio Minimum of 12 months of home health experience within the past 3 years Proficiency with HCHB (Homecare Homebase) documentation system Experience completing OASIS assessments Ability to meet productivity and hourly requirements Preferred Experience / Skills Strong organizational and time management skills in a field-based care setting Experience working independently in a home health or community-based environment Why Join Us 13-week contract assignment with a guaranteed 40 hours per week Opportunity to make a meaningful impact delivering therapy services directly in patients' homes
