Key Responsibilities

• Plan and conduct segments of treatment programs designed to restore and improve physical, social, and mental functions
• Meet department objectives while delivering high-quality patient care in a home health setting
• Serve as co-leader in an interdisciplinary approach to patient care and group treatment
• Provide physical therapy services across the Butte, Yuba, and Sutter County service areas

Required Qualifications

• Doctor of Physical Therapy (DPT) degree required for those who obtained licensure on or after January 2016
• Active Physical Therapy licensure in all applicable states of practice
• Current CPR or Basic Life Support (BLS) certification
• Valid Driver's License; must be at least 21 years of age or older

Preferred Experience / Skills

• Minimum of 1 year of prior home health experience preferred
• Experience with Home Care Home Base (HCHB) software
