Schedule / Job Type

Contract | Full-Time | Onsite | Monday through Friday, 8:00 AM – 5:00 PM | No on-call or weekend requirements

Key Responsibilities

• Provide home health physical therapy services to patients in assigned territories including Yorba Linda, Orange, Anaheim Hills, and Santa Ana
• Manage a caseload of approximately 6 patient visits per day
• Evaluate and treat patients in their home setting, developing and implementing individualized care plans
• Document patient progress and maintain accurate clinical records in compliance with home health standards

Required Qualifications

• Active Physical Therapist (PT) license in the state of California
• Minimum of one year of home health experience
• Ability to travel to patient homes within the designated service territories

Preferred Experience / Skills

• Experience with HCHB (Homecare Homebase) documentation software
