Key Responsibilities
- Completes comprehensive evaluations of assigned patients and develops, implements, and modifies individualized treatment plans in accordance with patient needs, goals, and physician direction.
- Completes medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines.
- Delegates and directs all discipline-specific therapy services for assigned patients, ensuring care is provided under signed, valid physician orders and within insurance approvals.
- Designs functional treatment programs to support effective patient return to community activities and progression toward discharge within an established length of stay.
- Participates in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations.
- Adheres to discipline-specific State Practice Act and all applicable regulatory agency requirements, maintaining professional licensure in good standing.
Required Qualifications
- Active and unrestricted Physical Therapist license in the state of California.
- Minimum of one year of clinical experience as a Physical Therapist; new graduates will not be considered.
- Proficiency in clinical reasoning, treatment planning, and goal writing within an outpatient setting.
- Knowledge of and adherence to state and federal documentation and compliance requirements.
- Commitment to patient and employee safety standards and safe patient handling practices.
Preferred Experience / Skills
- Experience in an outpatient rehabilitation setting with a focus on functional outcomes and community reintegration.
- Active involvement in continuing education, professional organizations, or self-directed professional development initiatives.
- Familiarity with performance improvement methodologies and quality improvement processes in a clinical environment.
