Key Responsibilities

• Evaluate and treat patients using medically prescribed physical therapy programs with the goal of restoring function and minimizing disability following disease or injury
• Conduct initial admission and evaluation visits, including completion and timely submission of OASIS scores to the assigned Clinical Team Manager
• Document observations, clinical findings, problems, skilled interventions and treatments, goals, and discharge plans
• Supervise Physical Therapy Assistants as applicable
• Provide services across an assigned territory including communities in the San Mateo County and surrounding areas

Required Qualifications

• Active Physical Therapist license in the state of California
• Minimum of one year of home health experience
• Proficiency in evaluating and treating patients within a home health setting
• Ability to complete OASIS assessments within established timelines
• Valid driver's license and reliable transportation for field-based patient visits

Preferred Experience / Skills

• Experience supervising Physical Therapy Assistants
• Familiarity with home health documentation and electronic medical record systems
